Personalized Services & Assistance
We understand that each resident is an individual with unique needs and preferences,
and we want to determine the appropriate level of care that preserves each resident’s
dignity, encourages independence and best enables freedom of choice. Prior to move-in,
we meet with each resident and family to conduct an assessment to understand the
need for assistance and support in the areas listed. Following the assessment, our
team prepares an individualized service plan that is regularly reassessed.
